BACK-TRACspinal decompression

Our approach

Evaluate first. Then treat, measure and adapt.

Every new patient starts with an exam at a free first visit, so we know whether spinal decompression is appropriate for your condition before anything else is decided.

  1. 01

    Evaluate

Determine what may be contributing to your symptoms.

  2. 02

    Treat

Begin appropriate nonsurgical treatment if you are a candidate.

  3. 03

    Measure

    Track your response to treatment.

  4. 04

    Adapt

Change the plan based on how you respond.

Evaluate

The first visit is a conversation, an exam and a reading of your imaging, in that order. We want to know when it started, what changes it, what you have tried and what happened. We examine the spine and the nerves that leave it: strength, reflexes, sensation, the movements that reproduce your pain. Then we look at your MRI or X-rays with those findings in front of us, rather than the other way around.

We are looking for agreement between your story, the exam and the picture. When they agree, we know what to treat. When they do not, we say so.

Treat

If spinal decompression is appropriate, your first treatment happens the same day. You are seated and reclined, and the table gently separates the vertebrae to take pressure off the disc and the nerve beside it, directed at the low back or the neck, with settings set for you. How it works, in plain English.

Measure

Before treatment starts we record the numbers that matter for your condition: pain at rest and with activity, how long you can sit or stand or walk, the nerve findings, the movements that are limited. Those same numbers are checked as care goes on. Progress is something we can show you, not something we ask you to take on faith.

Adapt

Treatment changes with your response. Settings change, the plan changes, and if the numbers are not moving we tell you and talk about what that means, including when a different kind of care is the right answer.

Who it is not for

Fractures, tumors, infection, severe osteoporosis, spinal instability and cord-level symptoms are not treated here. Part of the first visit is finding those and pointing you to the right care. If you've had a fusion or have hardware, we check how long ago and clear it with you at the first visit.

What happens at your first visit

  1. 1

    Consultation

With Dr. Metcalf: your history, what you've tried, and your MRI or X-rays reviewed with you.

  2. 2

    Exam

A focused spine and nerve exam, with a measured baseline, so progress is a number and not a feeling.

  3. 3

    First treatment

Your first spinal decompression treatment the same day, set for your lower back or your neck, so you feel what it does before you decide anything.

  4. 4

    A straight answer

Candidate or not, and if you are, a plan for what comes next.
